A treadmill test, also known as a cardiac stress test, is used to assess the heart's ability to withstand external stress in the form of exercise. It is one of the most commonly used tests in controlled clinical settings to diagnose ischemic heart disease and predict prognosis following myocardial infarction. Patients are examined to determine their pretest likelihood of becoming positive. A resting ECG is obtained. The gradual increase in speed and slope is explained to them. Vital signs are monitored before and during exercise. If the patient experiences chest pain or severe breathlessness, the ECG shows ST elevation, or the patient collapses, the test is immediately terminated. If the ST depression is greater than 2 mm or there is new onset ST depression in the recovery phase, the test is positive.
